Tadalafil is a prescription medication called a phosphodiesterase type 5 (PDE-5) inhibitor that increases blood flow (DailyMed, 2025).

It works by blocking the PDE-5 enzyme, which normally breaks down the chemical messenger (cyclic guanosine monophosphate – cGMP) to relax blood vessels. By slowing the breakdown of cGMP, tadalafil allows it to remain active longer, relaxing the smooth muscle in the blood vessels and penile tissue, boosting blood flow to the penis (and other areas), which makes it easier for the body to get and keep a natural erection during sexual stimulation (DailyMed, 2025).

Some other PDE5 inhibitors are short-acting; however, tadalafil has a half-life of approximately 17.5 hours, meaning it takes 17.5 hours for half of the medication to be eliminated from the body. Because of this, tadalafil can improve the ability to achieve an erection for up to 36 hours, which is why it's nicknamed "the weekend pill" (DailyMed, 2025).   • Kidney Problems: If the kidneys aren't working well, the body can't clear out medicine effectively, so it could build up in the blood, increasing the risk of side effects (DailyMed, 2025).
  • Moderate to Severe Hepatic Dysfunction: Liver disease can slow the breakdown of tadalafil and increase the risk of side effects. Therefore, a lower dose may be needed with moderate hepatic dysfunction; tadalafil is typically not recommended for people with severe liver disease (DailyMed, 2025).

  • Be Consistent: If you're on a daily dose, take it at the same time every day to keep medication levels steady.
  • Evaluate Your Hormones: If you have low testosterone, it may reduce your libido or cause other symptoms like erectile dysfunction. If you are experiencing symptoms of low testosterone, a medical provider might suggest hormone testing.
  • Don't Worry About Food: Food has little effect on tadalafil absorption, so it can be taken with or without a meal (Huang & Lie, 2013).
